DNR- ORANGE FORM

In Arizona an individual who is in the last stages of life and wants to refuse any resuscitation for cardiac or respiratory arrest can sign along with their health care professional a DNR (DO NOT RESUSCITATE) order.  This form is also referred to as an “ORANGE FORM”.   Post this orange form in plain sight so the paramedics or hospital emergency personnel know NOT to administer drugs, cardiac compression, endotracheal intubation, artificial ventilation, defibrillation, and related emergency medical procedures. They will NOT WITHHOLD medical interventions that are necessary to provide comfort care or alleviate pain.

It is important to consult with your health care professional before signing this DNR. Word of Caution-most patients who are not terminally ill will NOT want to direct that resuscitation not be attempted.
